Manager John Farrell said Holt had a “whiplash” type feeling after diving for a ball last week. After soreness in his neck subsided “some of these other symptoms of being a mild concussion were still present.”
To take their spots, catcher and recently converted left fielder Blake Swihart and right-hander Noe Ramirez were recalled from Triple-A Pawtucket.
The 26-year-old Smith made just three appearances since coming off the DL on May 3, allowing only an unearned run in 2 2/3 innings. His first stint on the DL was called a flexor muscle strain.
Holt, 27, is hitting .239 with three homers and 19 RBIs in 35 games.

The two-time NL Cy Young Award winner is attempting to come back from left hip surgery. Lincecum didn’t pitch after June 27 last year with the Giants — his only other major league team — because of degenerative hip problems.
He had surgery Sept. 3 and didn’t sign with a team after finishing a $35 million, two-year deal last season.
Lincecum’s free-agent deal with the Angels includes $1,175,000 in performance bonuses and $500,000 in roster bonuses. He would receive $25,000 for making 11 starts, $50,000 for 13, $100,000 for 15, $200,000 for 17 and $400,000 each for 19 and 21 starts.
In addition, he would receive $125,000 each for four, 30, 60 and 90 days on the active roster, excluding disabled list days spent due to a right hip injury.
The 31-year-old right-hander went 7-4 with a 4.13 ERA in 15 starts last season.

Rangers owner Ray Davis joined Arlington Mayor Jeff Williams in Friday’s announcement.
Davis says putting a roof on Globe Life Park, which opened in 1994, would be too expensive. He says a climate-controlled venue could attract more fans on hot, summer days. Davis says the Rangers, marking 45 years in Arlington, expect the new stadium to open in five years.
Voters in November are expected to be asked to support the project by continuing existing sales taxes that are used to help pay for A&T Stadium, the next-door home of the Dallas Cowboys.
The city’s commitment would be capped at $500 million.

Following the shortest outing of his career Thursday night in a 9-1 loss to the Nationals, the Mets left open the possibility of skipping Harvey’s upcoming turn in the rotation. But in a meeting with staff members Friday, manager Terry Collins said, Harvey insisted he wants to fight through his problems on the mound.
Harvey is slated to start Tuesday night, but Collins said that could change. The right-hander might be pushed back a day or even moved up to pitch on three days’ rest Monday because he threw only 61 pitches Thursday.
Harvey, an All-Star in 2013, is 3-6 with a 5.77 ERA in nine starts this season.
